Leah Cohen presentation.------------------------------------------------------------------- On July 2, 2004 the Legislative Counsel Bureau (LCB) approved the Standardized Initial Application NDOI 901 Form, as distributed (no changes), and usage of said form has been enacted into law. Per Section 1. Chapter 679B of Nevada Administrative Code (NAC): Each insurer, carrier, society, corporation, health maintenance organization and managed care organization shall use the Nevada Division of Insurance (NDOI) Form 901 prescribed by the Commissioner of Insurance pursuant to NRS 629.095 when obtaining any information related to the credentials of a provider of health care. Nevada hospitals are presently not mandated to use the NDOI Form 901; however, the Nevada Hospital Association has committed to the DOI that our membership hospitals will use the form voluntarily, in an effort to avoid future legislation mandating hospitals to do so. For your convenience, please find approved NDOI Standardized Initial Application Form 901. The form may also be accessed on the Nevada Division of Insurance’s website at www.doi.state.nv.us or on Saint Mary’s Regional Medical Center’s website at www.saintmarysreno.com; it will also be available very shortly on the Nevada Hospital Association’s at www.nvha.net.
